My guess is that the WP systems architecture probably involves some “Middleware” servers, which amount to great big hyperpowerful “Last In First Out” [LIFO] st@cks, and the Middleware servers know how much data they can reliably attempt to send to the backend Database servers at any point of time, and if at some point in time, too many kkk0mments are being sent to the LIFO “St@ck” of the Middleware, then the earliest kkk0mments [which lie at the “bottom” of The St@ck] are simply discarded.
Sometimes, after about six or eight hours, a kkk0mment will suddenly appear at the Frontend [which delivers the final product to us], so not all kkk0mments towards the bottom of The St@ck are always discarded, but lately it feels like the Middleware St@ck is being overwhelmed by something.

From what I see running a WP blog, comments that trigger the spam filter (either because of owner-set keywords or system algorithm), disappear as though they were eaten when you press Send. They go to the blogger’s Spam folder, and he doesn’t know they’re there unless he actively looks for them.
With CH’s traffic volume, he probably gets thousands of spam comments daily, making it too much of a drudgery to go look for legitimate comments that were caught there.
It always sucks when you (as a blogger) find a wrongly spammed days-old comment there. On my site, they tend to be long comments from a first-time commenter.

PA, the reason I had been hypothesizing an overburdened LIFO St@ck is because the kkk0mment-bouncing seems to be so strongly time-dependent.
There are times of day when everything goes through, and other times of day when nothing goes through, and that always felt to me like a LIFO St@ck in front of something which was collapsing under the weight of the traffic [at peak traffic times of day].
Maybe there’s a LIFO St@ck in front of a Spam Filter, and if so many kkk0mments are coming in that the Spam Filter’s CPU becomes overburdened in examining the kkk0mments, then the LIFO St@ck starts discarding the kkk0mments at the “bottom” of the St@ck. [Or maybe at peak load, the LIFO St@ck simply bypasses the Spam Filter altogether and pushes all of the “bottom” kkk0mments directly to the Spam Inbox.]
I can imagine that Spam Filters could be highly CPU-intensive – maybe even more CPU intensive than the relatively simple Writes-to-Disk which the database backend performs.
